Enter online by completing the form at http://www.familymedia.com/dmi or by sending an email with the required information to contest@familymedia.com. To enter via mail, send a postcard to “Heartfelt Sweepstakes, PO Box 399, Old Bethpage, NY, 11804.”
- Enter the “Heartfelt” Sweepstakes (the sweepstakes) by completing the online entry form available at www.familymedia.com/dmi/. Entries may also be mailed to2013 Heartfelt Sweepstakes, PO Box 399, Old Bethpage, NY 11804.
- Entries are limited to FOUR (4) per day (24 hour period), per household (as determined by IP address for electronic entries). Any entries after the first four per 24 hour period will be disqualified.
- Entries must be postmarked by March 15, 2013 and received by March 20, 2013. Include your name, address, and commissary name on each entry. Online entries must include a valid email address. Incomplete entries and electronic entries without a valid email address may be disqualified.
- The sweepstakes is open only to authorized military commissary patrons 18 years of age or older who retain commissary shopping privileges. Employees and their families of sponsoring companies and their affiliates, subsidiaries, brokers and personnel employed in the military commissary or exchange systems are not eligible. Program void where prohibited by law. Offer subject to all federal, state and local rules.
- Sweepstakes winners will be randomly selected selected from entries received. Odds of winning will be determined by total entries received. Not responsible for lost or misdirected entries.
- 100 winners will be selected to each receive one $50 commissary gift card.
- Winners will be notified on or before April 15, 2013. Prizes not claimed within 30 days of notification will be forfeit and an alternate winner will be selected at random.
- Winner will be required to furnish an eligibility affidavit of eligibility as proof of being an authorized commissary shopper.
- All entries become the property of FamilyMedia. Entries cannot be returned. All judges decisions are final.
- Any taxes shall be the sole responsibility of winners.
- Upon acceptance of prize, all winners must agree to publicity photos if requested.
